Together with PharmAccess, an NGO focused on improving healthcare in sub-Saharan Africa, we’ve implemented an integrated patient-centered model of care aimed at improving diagnosis and disease management for patients with cardio-metabolic diseases. The care bundle consists of access to patient group meetings, digital self-management support, remote care, and medications. We have established 32 active community-based NCD remote care groups involving over 1,400 patients, making NCD services and support more accessible. More than 900 patients have used the call center for telehealth support, with medications consistently available (99%) to patients within patient groups. Overall, we’ve seen approximately 40% improvement in disease control among participants.
In light of the program’s success, the Ministry of Health aims to expand this community-based management model to all districts and facilities with NCD clinics in Zanzibar. PharmAccess will coordinate the handover to district health management teams with integration of CHWs, ensuring sustainability of the model within the Zanzibar health system.

Our CommonHealth partnership in Myanmar has broadened access to medicine and patient services through increased capability building for healthcare professionals, specialist engagement, family medicine service, and teleconsultation. Within 18 months, approximately 33,000 people with diabetes and/or hypertension enrolled in the patient support program, with more than 17,000 patients utilizing the family medicine service. The program aims to increase the share of active telemedicine users who have their diabetes and/ or hypertension under control by 19 percentage points. Over 150 HCPs have been trained and onboarded into the program to provide improved service to patients, enhancing the quality of care available in the region. CommonHealth is southeast Asia’s first e-commerce platform dedicated to people with diabetes, hypertension, and other long-term health needs.
