In Guinea, we’ve partnered with Santé En Entreprise, a French NGO specializing in workplace and community health, to support the decentralization and capacity building of diabetes and cardiovascular disease management units. Through the integration of digital solutions in the healthcare system, we’ve helped facilitate more efficient patient management and data collection. Our program has trained over 350 HCPs from 91 health centers to better diagnose NCDs. We provided 8 regional health facilities with digital solutions to optimize NCD patient management, resulting in over 14,000 digitalized patient records. The screening campaigns have also reached more than 20,000 people , raising awareness about NCD risks and prevention strategies.
Looking forward, we’re expanding this successful model to Chad, where we’ll continue to support health system strengthening through training, awareness campaigns, and digitalization of care services for patients with NCDs.
Diabetes screening session in the Democratic Republic of the Congo in collaboration with Programme National de Lutte Contre le Diabète and SANRU
Our partnership with Health Builders in Rwanda, an NGO focused on strengthening primary health systems, has focused on empowering CHWs for early detection and management of hypertension and diabetes in Nyabihu District. Through this initiative, we’ve made communities more aware of NCD risks and prevention strategies, with over 200,000 people screened. Approximately 43% of diagnosed patients are now linked to care in follow-up programs, ensuring continuity of treatment. Around 1,800 CHWs have been trained on prevention, screening, and follow-up, with about half receiving ongoing mentoring to support patients effectively. The program’s sustainability is enhanced by the use of electronic medical records on CHWs’ smartphones, improving data collection and patient monitoring.
